Who We Are
Chances ADP, LLC is a community-based adult day program serving adults with intellectual and developmental disabilities (IDD). We operate a without-walls model, meaning participants spend their program days in the community rather than in a fixed facility.
Our program focuses on person-centered care, fostering independence, social integration, and skill development. We maintain low staffing ratios (1:2 and 1:3) so every individual receives consistent, individualized support.
Our Mission
To empower adults with intellectual and developmental disabilities to achieve their full potential through community participation, positive behavior support, and meaningful daily experiences that promote choice, dignity, and belonging.
How We Work
- Community-based (Without Walls): Activities take place in real community settings—parks, grocery stores, libraries, cafes, volunteer sites, and local events.
- 1:2 & 1:3 Staffing Ratios: Low ratios allow for personalized attention, safety, and genuine relationship-building.
- Door-to-Door Transportation: Reliable transportation to and from the program and community activities.
- Behavior Support: We implement individualized behavior support plans with a focus on dignity, choice, and reducing barriers to participation.
- Person-Centered: Each participant’s interests, strengths, and goals guide daily activities and long-term planning.
- HCBS-Aligned: Our practices support Home and Community-Based Services principles of integration and autonomy.
Our Core Principles
We center each individual’s dignity, preferences, and well-being in everything we do.
We document progress and share meaningful moments that help families, regional centers, and the community understand impact.
Every outing, every interaction, and every day reflects professionalism, respect, and high standards of care.
Who We Serve
Adults with intellectual and developmental disabilities who benefit from structured community-based support, behavior support, skill-building, and social participation. Services are provided under regional center vendorization (Service Code 532 behavior management day program).
In the Community
Chances ADP is built around real community participation. Participants spend their days in everyday places — stores, parks, restaurants, campuses, and local attractions — building skills and relationships where they live.
